Echo of Varshan Location & Summon Requirements
The Echo of Varshan boss fight can be found inside the Malignant Burrow. This dungeon is really easy to find as it is right next to the Tree of Whispers in the Writhing Mire zone in Hawezar. When you enter it, you will have to fight a small number of enemies before you finally reach the summoning altar.

Malignant hearts can be found by opening the cache you receive from completing Grim Favors for the Tree of Whispers in any of the new Torment difficulties. Another way of obtaining these items is by defeating Grotesque Debtors, which have a chance to spawn after completing Whispers of the Dead quests around the open world. You can also earn Malignant Hearts by defeating Elite monsters and World bosses, as well as through the new Undercity of Kurast dungeon and bartering with your Mercenaries.

How to Summon the Echo of Varshan
The Echo of Varshan can only be found and summoned while playing on any of the new Torment difficulties introduced in the Vessel of Hatred expansion. This means that you will need to reach level 60 and complete a Tier 20 run of the Pit before you can summon him. Higher Torment difficulties will make the fight much harder, with an increased life pool and damage for him, while your character receives penalties to armor and resistances. However, defeating him on higher difficulties will be much more rewarding, with more Unique items dropping and an increased chance of getting Mythics.

Varshan also has a small chance to drop Uber Unique items himself, making the fight much more rewarding in the long run.
